Anonymous Options Listed on Same Page

Right now the two options to be anonymous (i.e. Amount and Display Name) are listed on separate pages. Additionally, if a choose a display setting and then skips the message page, their preference/s are not saved. We would like to see the Anonymous options - Amount, Display Name, Hide Name/Photo appear together on the first page of the donation form.
